Job Purpose:
The Caseworker will provide grassroot case management support to children with specific needs through case identification, assessment/registration, documentation, Case planning, case plan implementation (including direct and indirect services of referrals and FTR) follow- up/ monitoring and case closure for UASC & CR children.
