Job Purpose
• Through your initiative you will make an important contribution to Imprint Hope’s work to serve children with disabilities and their families. As the first-line representative of Imprint Hope to the families and communities we serve, you will have primary responsibility to implement or coordinate community-based and home-based programs and services. You will work collaboratively with IH’s multi-disciplinary team to ensure each beneficiary family receives holistic high quality, responsive and impactful interventions in their home, especially those caregivers graduating from Imprint Hope’s 7-week residential training program. In addition, you will lead implementation of community-focused programs to raise awareness, reduce stigma, and increase support for families of children with disabilities. Your understanding and passion for Imprint Hope’s Mission will help the organization to develop partnerships with local government officials and other key community stakeholders.

Duties And Responsibilities

Key Responsibilities
• Ensure effective referrals of potential Imprint Hope beneficiaries in the field.
• Build relationships and coordinate pre-intake assessments with potential beneficiary

• Conduct follow-up visits with IH Caregivers who graduate from 7-week training program; communicate proactively with interdisciplinary team to inform intervention plans and coordinate timely services from other team members.
• Under direction of the Psycho-Social Support Counselor, lead implementation of community-based support groups for supported IH caregivers. Ensure high-quality implementation of the established curriculum and methodology, including reporting as required.
• Work with the IH team to coordinate community Education/Sensitization Sessions and other community activities.
• Contribute to inter-disciplinary review and intervention planning for IH beneficiary families.
• Contribute to implementation of Imprint Hope Monitoring and Evaluation System, ensuring accurate and timely reporting of community and home-based services. This may include providing liaison with District offices to ensure required reporting per established protocols and timelines.
• Appropriately receive feedback from IH beneficiaries and share with IH team or escalate as appropriate.
• Initiate and build partnerships with key stakeholders to advance Imprint Hope’s mission in areas we serve.
• Build and maintain strong relationships with Local Government officers at district and community levels, including LCs, VHTs, and CBR Officers.

Qualifications & Experience
• Bachelor’s degree in social sciences, public health, social work or community health and development is required.
• Minimum of 3 years of work experience leading delivery of community based social or health services within development/humanitarian programming.
• Experience focused on children and families is a plus
• Experience with rural or highly vulnerable populations is a plus
• Fluent English and Luganda speaker
• Basic Computer Skills – typing, email, Microsoft Word and Excel,
• Basic English-writing skills (ability to write basic sentences and paragraphs for emails,
• reports etc)
• Experience working with or knowledge about persons with disabilities is a plus

• Empathetic, good observation, active listening and analysis skills with ability to make sound judgment.
• Good relationship management skills and the ability to work closely with local partners and community members.
• Attention to details, accuracy and timeliness in executing assigned responsibilities.

• Service-oriented, with passion and commitment to provide high-quality programs for the most vulnerable populations, especially persons with disabilities
